This case study was produced for a major tanker operator. One of their ships was involved in an incident that could easily have become a major accident of catastrophic proportions, with the potential to cause enormous harm to the company and the environment.
The purpose of the Case Study was to disseminate the lessons learned from the incident to their seagoing workforce and demonstrate to the vessels charters that corrective action was taken.
We were given the task of analyzing the events leading up to the incident. From this we developed a re-enactment of the ships passage and the actions of the Bridge Team. The various causal factors were identified and examples of best practice presented.
